Request an appointment

A member of our intake team will follow up within one business day. All clinical details will be gathered privately through our secure patient portal — not this form.

Please do not include sensitive health information (PHI) in this form. Communications through this website are not encrypted and are not HIPAA-secured. For confidential communication, please use our secure patient portal.

Your information stays with our intake team and is never sold or shared.

By submitting, you agree to be contacted by our intake team. You are not creating a clinical record.

Frequently asked questions

How long does treatment usually last?

It depends. Some people benefit from short-term, focused work over several weeks; others prefer longer-term therapy. Your provider will discuss what fits your goals.

Is my information kept private?

Yes. All clinical communication happens through our HIPAA-compliant patient portal. What you share in sessions is confidential, with limited legal exceptions your provider will review with you.

What are your office hours?

Monday through Friday, 9:00am–6:00pm. Saturday and Sunday appointments are available by request.

Can I do everything virtually?

In most cases, yes. Telehealth is available for Maryland residents whose clinicians are licensed in-state.
